Community Health Workers (CHW) bridge the gap between health care providers and populations in need of care. CHWs are trusted members of and have a close understanding of the communities they serve. In 2012, the Community Health Worker Initiative, led by the Kansas City CARE Clinic and other stakeholders, began to more fully incorporate community health workers into their health care delivery system.
