Safety clinics recognized as patient-centered medical homes

National efforts encouraged integration and improved quality at the local level. The patient-centered medical home (PCMH) model is a way to improve health care by transforming how primary care is organized and delivered. A number of safety net clinics in Kansas City have become recognized by the National Committee on Quality Assurance as patient-centered medical homes. Additionally the health home partnership with the State of Missouri began to specifically target those with complex chronic conditions.
